What is EVAF?

Electronic Visa Application Form DS-156 is mandatory for all visa applicants. Please note that this form must be electronically completed - we do not accept handwritten application forms.

All male applicants between the age of 16 and 45 are also requested to Complete Form DS-157

All student (F1) and exchange visitors (J1) are requested to complete Form DS-158

How does EVAF work?

  1. Enter the information requested into the appropriate spaces on the form. Please answer all the questions.
  2. Review the information you entered for accuracy.
  3. Press the continue button at the end of the form.
  4. Print the form after it is returned to you and displayed in Adobe Acrobat.
  5. The bar code that prints on the last page contains the information you entered and will help us process your application faster.
  6. Bring all printed pages with you when you come in for your interview.

     

Note: Any inkjet or laser printer should work for the purpose, but a dot matrix printer would not print at the sufficient resolution.
